Well, I don't think that being a substitute shinigami comes with a package of list of things to do for a regular shinigami. If he held such responsibility and should have altered his course of actions to be seen as a "good" shinigami, then I'd very much believe that Old Man Yama would have held him by his collar and prevented him from re-entering the human world.
Ukitake said that a badge is given to spiritual beings, who may be deemed as being helpful towards Soul Society and declared to be a substitute shinigami. I don't think he has to do something, but just help out.
After SS arc, until he got interrupted by arrancars, Ichigo was slaying hollows around his town. I don't think he was neglecting his duties.
He was ripping off the entire handbook, which stated SS' rules and regulations, only before he was officially accepted by captains as a substitute shinigami. Apparently, they accepted his actions, as he helped SS survive through a crisis.
Then, he disobeyed one order, which was to not to enter Hueco Mundo to save Orihime.
Considering Ichigo's backstory as a human and how he's come to become a shinigami, I'd believe he's a good shinigami. He attained high level shinigami techniques like changing your toilet papers. Also, he did manage to risk his life in slaying hollows, so I believe that's a good thing. He's a teenager and hasn't been disciplined as a shinigami. It's highly likely that one would go off getting revenges and use his/her shinigami powers in doing stupid stuff. Ichigo was well behaving. Generally, I believe he's on the good side.
